Services Available

Area Agency Funded Services:
Congregate Program 10 Meal Sites in two counties providing a hot meal at noon and daily activities such as, live music, dancing, educational presentations and much more.
Home Delivered Meals provides hot & cold meal to homebound seniors
Case Coordination & Support Once referred to us,  a Case Coordinator will meet one on one with the client in their home to discuss services available, coordinate services and periodically visit the client's home.
Personal CareProvides assistance with personal hygiene needs
HomemakingProvides assistance with light housekeeping such as dusting, vacuuming and grocery shopping.
RespiteProvides relief and time out for primary caregivers so they can run errands, attend doctors appointments or just rest.
Caregiver and Disease Prevention - Provides training resources for caregivers, disease prevention information and health promotion activities to seniors.
 

 

Other Programs we offer:

Medicare/Medicaid Assistance Program – Assists Seniors with eligibility requirements for these programs as well as helping the Seniors resolve a wide variety of  health bill issues.
EPIC Program – Elder Prescription Insurance Coverage –(replaced the Michigan Emergency Pharmaceutical Program) – Provides prescription coverage to qualified low income Seniors.  Enrollment is limited to this program  
Medicaid Waiver –Area Agency and  A & D Home Health Care contract with our agency to provide services to clients  
Friendly Driver Program – Provides transportation to Doctors that are located out of county with Volunteer drivers.
File of LifeProvides a place for documentation of health and family contacts for emergency personnel. The file of life is a magnetic folder that is placed on the client's refrigerator. Quite often our Case Coordinators assist the Senior in filling this out.
United Way Emergency Needs – Provides an avenue for our Seniors when they are faced with an emergency need such as hot water heater, hearing aids, fuel gas, dentures, eyeglasses. 
County Prescription Discount Card – Made available through Clare & Gladwin County Governments and administered by Council on Aging. This card offers uninsured Seniors 60+ discounted prices on brand name and generic drugs. It is honored at most area Pharmacies. 
Patient Assistance– Assist the Seniors in finding coverage for their prescriptions through various Pharmaceutical Companies.
